#10 – Mix liners, bases, cements, composites, amalgams

A. Liners

1. Used to prevent chemical irritation to teeth, reduce micro leakage, reduce thermal sensitivity, adhesion of filling to teeth, force pulp to generate dentin to seal exposure.
2. Very thin 15mm thick
3. a .5mm thickness of dentin reduces material toxicity by 75%
a 1mm thickness of dentin reduces material toxicity by 90%
4. SE Bond is currently used
5. Resin modified glass ionomere (RMGI)
a. Used to protect a material used to stimulate dentin
b. Advantage
i. Bonds to dentin
ii. Bonding agent not required
iii. Releases fluoride
iv. Easy to use
c. Disadvantages
i. Slow set (several hours)
ii. Low strength
iii. Poor color match
iv. Harder to place than flowable
d. Technique
i. Place 1 layer .5 mm thick care
ii. Self-etch bonding agent
iii. Place resin filling
iv. Finish, polish

B. Bases

1. Gerestore is light cured calcium hydroxide
a. Used as a direct or indirect pulp cap to generate dentin to protect the pulp. Also used to reduce thermal sensitivity.
b. Must be covered
2. Flowable resin
a. Advantage
1. Easy to use
2. Tooth colored
b. Disadvantages
1. No flouride

C. Cements

1. Used to cement crowns and bridges
2. Temporary
a. Temp bond
b. Temposeal

3. Permanent
a. Resin
1. excellent for single crowns
2. adhere to dentin, low solubility, high strength, reduce leakage
3. have fluoride release for 5-7 years – reduce sensitivity
b. Used to cement single crowns and short bridges

b. Technique
1. Dry tooth
2. Place one click/town on mixing pad
3. Mix for 10 seconds
4. Coat crown
5. Doctor will seat
6. Light cure facial and lingual- 2 seconds each
7. Doctor will remove excess cement (floss for proximals)
8. Light cure facial and lingual- 20 seconds each
9. check for any excess cement (floss proximals)
10. Check occlusion

b. PhotacFil
1. Best adhesive restorative
2. Poor color match
3. Technique
a. when decay removed, mix for 8 seconds in amalgencetor
b. dentist will place with an explorer
c. light cure for 20 seconds
d. finish with ET’s
e. for improved esthetics, use sandwich technique: cover PhotacFil with SE Bond and then Gradia

c. Panavia
1. Low solubility, high strength, bond to tooth, reduce sensitivity, reduce leakage
a. Expensive require anesthesia (tooth must be rinsed), takes more time, short working time
b. Either chemical, light or dual cure
2. Panavia F
1. Uses
a. Cementing posts in endo teeth
b. Cementing etch bridges
c. Cementing long span bridges
2. Materials all included in the kit make sure all spaces in the kit are filled
